Transesophageal echocardiography to help position and deploy a transcatheter heart valve.

چکیده
RANSCATHETER AORTIC VALVE IMPLANTATION (TAVI) IS A CLINICALLY ACCEPTED PRACICE IN EUROPE for patients with high or prohibitive surgical risk and is undergoing clinical investigation n the U.S. and Canada. Some investigators have utilized transesophageal echocardiography (TEE) to select ranscatheter heart valve (THV) size, monitor the procedure, and identify complications (1,2). The use of TEE o position and deploy the THV has been limited by the ability to image the THV within the calcium hadowing of the native aortic valve, and the ability to differentiate the THV from the balloon on which it s mounted. In this iPIX, we show the use of 2and 3-dimensional TEE to aid in the positioning and eployment of THV. If the appropriate technique is used, TEE can be an important adjunct to fluoroscopy nd, therefore, improve the safety and efficacy of the TAVI procedure.

Transesophageal echocardiographic guidance of transcatheter closure of the aortic valve in a patient with left ventricular assist device-related severe aortic regurgitation
A 68 year-old man with a severe ischemic cardiomyopathy underwent left ventricular assist device (LVAD) implantation (Heart Mate II device) for destination therapy. He presented 49 months following LVAD implant with worsening heart failure symptoms and new severe aortic regurgitation. Given high risk for both surgical and transcatheter aortic valve replacement, he was admitted for transcatheter...
متن کاملInfluence of Echocardiographic Guidance on Positioning of the Buttoned Occluder for Transcatheter Closure of Atrial Septal Defects.
Ideal position of the buttoned device for occlusion of atrial septal defects was achieved more often with transesophageal than with transthoracic echocardiographic guidance (10/11 vs 7/23; P = 0.001). Patients with ideal device position were less likely to have residual shunts, device unbuttoning, or atrioventricular valve regurgitation (2/17 vs 11/17; P = 0.002). We therefore recommend the use...
متن کاملMalfunctioning mitral valve-in-valve bioprosthesis assessment by 3-dimensional transesophageal echocardiography.
An 85-year-old woman presented to the emergency department with symptoms of acute heart failure. The patient had an extensive cardiac history, including a bioprosthetic mitral valve replacement (Mosaic II, number 2) in 1994 and a transcatheter transapical Edwards SAPIEN THV 23 mitral valve implant within the mitral bioprosthesis (valve-in-valve implantation) in 2010 because of bioprosthetic val...
متن کاملEvaluation of transesophageal echocardiography in detecting cardiac sources of emboli in ischemic stroke patients
Background: Embolus is one of the causes of ischemic stroke that can be due to cardiac sources such as valvular heart diseases and atrial fibrillation and atheroma of the aorta. Transesophageal echocardiography (TEE) is superior in identifying potential cardiac sources of emboli. Due to insufficient data on TEE findings in ischemic stroke in Iran, the present study was done to evaluate TEE in...
